It is a satisfying story. It is also the part that the strongest studies have had the most trouble showing. What follows is what the research actually put in front of people, what it found, and what it openly admits it could not rule out.

What do people with rheumatoid arthritis actually report about brain fog?

We read six first person accounts published under their authors’ names across two patient platforms, plus a clinician reviewed community page carrying five unidentified member quotes. They can show what someone experienced, but they cannot prove what is happening inside another person’s head. They are worth reading anyway, because the pattern in them is not the pattern the ranking pages describe.

Writing for CreakyJoints in May 2020, the patient advocate Eileen Davidson described losing the thread of her own sentence, reading a page two or three times before it went in, and missing appointments outright. She linked it to flares, poor sleep and taking on too much, rather than to any single cause. An earlier CreakyJoints piece from September 2018 quoted two women on the same symptom: one in San Antonio on how other people look at you when you lose your words, one in Tacoma on staying inside what she calls her energy envelope and resting until it passes.

On RheumatoidArthritis.net, Mariah Z. Leach, ten years into RA, wrote the sentence that the research literature has been circling for a decade: she cannot tell whether her fog belongs to fatigue, to inflammation, to having three small children, or to all of it at once. On the same site, Leanne Donaldson gave up multitasking entirely and reported that block scheduling and a notebook for stray thoughts made her days workable, and David Advent described grogginess, headaches and draining energy that got worse during a stretch of isolation. The myRAteam page, updated in October 2022, carries members saying they feel they are losing their minds and that they now live by calendars and notepads.

Here is where the accounts and the guidance part company. Every account that names a cause names several at once. Not one of them describes the clean single mechanism story. Where they do agree with the official advice is on the practical side: sleep, pacing and writing things down come up repeatedly. Where they cut against the pattern matters too. Donaldson reports a workaround that held. Davidson credits getting her disease better controlled. Nobody in the set reports the fog simply going away, and none of these reports can be checked against a cognitive test.

How common is cognitive trouble in rheumatoid arthritis?

Depending on which paper you open, somewhere between nothing and three quarters of people with RA have measurable cognitive impairment. That is not a typo, and it is the most honest sentence in this article.

The 2018 systematic review by Meade and colleagues in Arthritis Care and Research screened 1,980 titles down to 15 studies covering 749 adults with RA. Only three of those fifteen reported a prevalence figure at all, and those three reported 30 percent, 0 percent and 31 percent. The authors concluded plainly that prevalence could not be determined. A study finding nobody impaired is not a result you will see quoted in a listicle.

A 2024 meta-analysis in The Clinical Neuropsychologist pooled 22 studies and got 49 percent overall. Split by instrument, that became 75 percent using the Montreal Cognitive Assessment, 56 percent using the Mini Mental State Examination, and 26 percent using a full neuropsychological battery. The measurement method was one of the statistically significant moderators. Put bluntly, the screening tool is doing a good deal of the work that the disease is being credited with.

What did each study actually measure, and what could it not rule out?

Ranking pages tend to hand you a number. Draw the line at 27 out of 30 and 72.2 percent are impaired. Draw it at 26 and 45.7 percent are. A third of the problem disappeared because somebody moved a threshold by one point. Any headline percentage that arrives without its cut off attached is not telling you very much. If that kind of measurement wobble interests you, we took the same approach to the evidence in our look at urinary tract infections and confusion. The same thing happens in lupus, where one Toronto cohort scored the same tests two ways and got 30 or 42 percent, as our comparison of lupus brain fog estimates by definition shows.

Does inflammation explain rheumatoid arthritis brain fog?

This is the claim doing the heavy lifting everywhere else. The Arthritis Foundation puts it as chronic inflammation, driven by cytokines such as tumour necrosis factor, changing the way the brain functions. To its credit, that page also flags a large study that found no difference at all, which many others do not.

That null result is worth meeting properly. Booth and colleagues, writing in BMC Rheumatology in 2021, used a nationally representative sample of 4,462 older US adults rather than a rheumatology clinic waiting room. Cognitive impairment was present in 36.7 percent of those with RA and 34.0 percent of those without, an adjusted odds ratio of 1.08 with a confidence interval spanning 0.74 to 1.59. No association. Their explanation for why they disagree with the smaller studies is uncomfortable and convincing: recruiting from clinics over-represents impairment in the RA group and under-represents it in the comparison group.

The 2025 Biomedicines study pushes in the same direction from a different angle. Across 62 adults given an 18 test battery, pain at the moment of testing tracked with poorer visuospatial memory, slower reaction times and reduced sustained attention, and the number of disease flares tracked with memory too. Erythrocyte sedimentation rate, the inflammation marker, showed no clear association with cognitive performance. So the sources genuinely contradict each other, and the honest summary is that something real is being measured while the mechanism remains an open question. Pain, disrupted sleep, fatigue and low mood all independently affect concentration, which is why fatigue and fog are so hard to pull apart in any condition, and why sleep keeps turning up as a confounder.

Could rheumatoid arthritis medicines be part of the picture?

The UK’s National Rheumatoid Arthritis Society, whose page was last updated on 15 May 2025, lists medication among six possible contributors alongside pain, fatigue, poor sleep, chronic inflammation and stress, and says some RA medicines have side effects linked to brain fog including confusion and mood changes. The Meade review raises methotrexate and glucocorticoids as candidate mechanisms, and hedges both.

The evidence is not settled either way. The 661 person UK cohort found no significant association between MoCA score and medication type, glucocorticoid use or anti-inflammatory use, while disease activity did show an association. Nobody should stop, reduce or change a prescribed medicine on the strength of an article, this one included. If you suspect a link between a medicine and how you are thinking, that is a conversation to have with the prescriber, who can weigh it against what the medicine is doing for your joints.

When is this worth raising with a doctor?

Brain fog is a description, not a diagnosis, and no article can tell anyone what is behind theirs. Speak to a doctor if thinking or memory problems are new, worsening, or interfering with work, driving or managing medicines; if they come with symptoms such as persistent low mood, confusion, or difficulty with familiar tasks; or if you are worried. Bring specifics, because dates, examples and how long it has been going on are more useful in a ten minute appointment than the word fog.

Clear communication with your healthcare provider helps identify the best management strategies.

It is also worth knowing that cognition is not routinely assessed in RA follow up, so it usually has to be raised rather than found. Multiple sclerosis care is further along on this point, since the NICE guideline says cognition should be assessed at the MS review, as our page on what MS cog fog is and which thinking skills it affects explains. Other conditions are linked to fog through better understood routes, and it is reasonable to ask what else is worth checking. Our overview of the causes worth ruling out first and the piece on coeliac disease and brain fog both cover ground a GP can act on.

Frequently asked questions

Is brain fog an official symptom of rheumatoid arthritis?

No. Brain fog is a plain language description rather than a clinical term, and it is not part of the diagnostic criteria for RA. It is widely reported by people with RA and by RA charities, and it has been studied using standardised cognitive tests, but the research measures cognitive impairment rather than a symptom called brain fog.

Why do the percentages vary so much between articles?

Because the studies used different tests and different thresholds. A 2024 meta-analysis of 22 studies found 75 percent impairment using the MoCA screening test, 56 percent using the MMSE and 26 percent using a full neuropsychological battery. One UK cohort of 661 people moved from 72.2 percent to 45.7 percent by shifting the cut off a single point.

Does rheumatoid arthritis affect memory specifically?

The 2018 systematic review found that adults with RA underperformed controls particularly on verbal function, memory and attention. Memory was the domain most commonly measured, in 13 of the 15 studies, so it also had the most opportunity to show a difference. Orientation was not measured by any of the included studies.

Is there a test that settles whether someone has cognitive impairment?

There is no single test. Screening tools such as the MoCA and MMSE are quick but were designed to flag people for further assessment rather than to diagnose, and full neuropsychological batteries consistently find lower rates than screeners do. Any assessment is a matter for a clinician, not an online quiz.

Do all studies agree that rheumatoid arthritis is linked to cognitive problems?

No. A 2021 study of 4,462 older US adults in a nationally representative sample found no association, with cognitive impairment in 36.7 percent of those with RA against 34.0 percent without. The authors argue that clinic based recruitment in smaller studies inflates the apparent difference.

Does controlling rheumatoid arthritis disease activity help cognition?

The UK cohort of 661 adults found that moderate and high disease activity were associated with worse cognitive scores than being in remission, which is suggestive but cross sectional, so it cannot show that one causes the other. No study reviewed here tested whether reducing disease activity improves cognition over time.

This article is general information, not medical advice, and is not a substitute for care from a qualified healthcare professional.
