This is the whole trick, and it is not even a subtle one. The strongest result a seller can point to is COSMOS-Web, which followed 3,562 older adults for three years and found better memory test scores in the multivitamin group. That is a real finding from a real trial. It is also a finding about memory scores in people over 60 measured across years, which is not the same question as whether a 34 year old who cannot hold a sentence in their head this month will feel clearer by next month.

Go down the list and the pattern holds. The Ginkgo Evaluation of Memory study randomised 3,069 people aged 72 to 96 and followed them for a median of 6.1 years: no effect on cognitive decline. NCCIH, reviewing the field, reports no convincing evidence for omega-3 supplements, and that short term studies suggest B vitamins do not help cognition in adults over 50. None of these studies asked anyone whether their head felt clearer.

Has any supplement actually been tested against brain fog itself?

One is being tested right now, and the detail is worth sitting with. A trial registered as NCT06695910 is measuring an omega-3 preparation against a Brain Fog Scale as its primary outcome, in people who report haze or mental fatigue regularly. It is randomised, placebo controlled and double blind, which is exactly right.

It also has 48 participants, it is sponsored by the company that makes the product, it began recruiting in November 2024, and its estimated completion is May 2027. Checked today, 23 September 2026, it is still recruiting. So the honest state of play is that the first proper trial of a supplement against brain fog itself has not finished, and when it does it will be a 48 person study funded by the seller. Every shelf of products promising clarity by Friday got there ahead of the evidence, and is in no hurry to wait for it.

Marketing claims on dietary labels often outpace the actual scientific evidence.

Why can a label promise focus without any of this?

Because it is allowed to, by design. Under the Dietary Supplement Health and Education Act, a supplement may carry a structure or function claim, which describes an effect on the normal structure or function of the body. Supporting cognitive function is such a claim. Treating brain fog would not be, because that edges into disease territory.

The rule is 21 CFR 101.93, and it is worth reading once. The manufacturer must notify the Food and Drug Administration no later than 30 days after first marketing, and must certify that the notifying firm holds substantiation that the statement is truthful and not misleading. Read that again and notice who is certifying. The claim then carries the disclaimer you have seen a thousand times without reading: this statement has not been evaluated by the Food and Drug Administration, and this product is not intended to diagnose, treat, cure, or prevent any disease. That is not legal throat clearing. It is printed on the packaging because nobody checked.

What happens when a regulator does check?

The clearest answer on record involves apoaequorin, the jellyfish protein sold as Prevagen. The FTC and the New York Attorney General brought a case in 2017. The substance of it was not that the company had no trial. It had one. The problem was that the trial failed to show a statistically significant result for the study population as a whole, and the marketing leaned on subgroups found afterwards.

After seven years of litigation and a jury trial, the FTC announced in December 2024 that the court had ordered the maker to stop making claims that mislead Americans concerned about memory loss. The case is not finished: an appeal was argued at the Second Circuit on 26 February 2026, covering the scope of the injunction and whether monetary relief was wrongly denied, and as of this writing no decision has been issued. Checked 23 September 2026.

Note what that story does not contain. It does not contain a rogue operator on a bad website. It contains a product sold in ordinary pharmacies, with a real clinical trial, advertised in a way a court found misleading anyway.

So which claims survive, and which do not?

Sorted by what was measured rather than by what is promised, because those are different columns and the marketing depends on you not noticing.

What is sold

What the quoted trial measured

What it found

Multivitamin

Memory test scores in adults over 60, tracked for 3 years

A benefit was found. COSMOS-Web followed 3,562 older adults and reported better memory scores in the multivitamin group. If you are genuinely deficient in something, correcting that deficiency can help, and that is a different proposition from buying a focus blend. Iron, B12 and thyroid function are the usual suspects, they are checked with a blood test rather than a hunch, and we have gone through which vitamins have evidence behind them and which do not.

The distinction is simple. Topping up a measured shortfall is treating a known problem. Taking a proprietary blend because your head feels thick is buying a lottery ticket with a monthly subscription. Individual ingredients get the same treatment here: see what the creatine trials actually show and the dose problem behind green tea and focus.

Sorting which kind of fog you have costs nothing and takes about a minute.

What is worth doing instead?

Working out which kind of fog you are dealing with, because the sensible next step is not the same for everyone. Fog that arrived after a virus, fog that tracks a hormonal cycle, fog that has followed eighteen months of stress and fog that turns out to be five hours of sleep are different situations wearing the same word. Our piece on the causes worth checking first covers the sorting.

There is a free six question assessment on the Brain Fog Remedy home page that does the same sorting in about a minute. It is not a diagnosis and it does not pretend to be one: it organises what you report into one of five patterns and matches a fourteen day plan of daily actions to it, with the plan itself sold separately. The reason it exists is the same reason this article does. Behaviour you can change beats a capsule whose evidence is still recruiting. If you would rather just start, what works and what does not for clearing fog needs no email address.

When is brain fog worth taking to a doctor?

Fog that is getting steadily worse, fog with memory loss that other people have noticed, fog alongside low mood, and fog with any new neurological symptom all need a doctor rather than a supplement aisle. So does fog severe enough to affect your safety at work or driving. Deficiencies and thyroid problems are found with tests, and are worth ruling out properly before spending anything.

Frequently asked questions

Do brain supplements work for anyone at all?

if you have a diagnosed deficiency, treating it may help.
